The Effect of Bone Graft in Treatment of Three Part Fracture of Proximal Humerus using Locking Compression Plate

Abstract
Purpose: To evaluate the effect of bone graft and results of treatment for three part fracture of proximal humerus using Locking Compression Plate (LCP).

Materials and Methods: Between January 2005 and June 2008, 23 patients with three-part fractures of the proximal humerus were treated by LCP fixation. Bone graft was not done in 15 cases and done in 8 cases. Their average age was 50.1 years (26~72 years). Postoperative mean follow-up period was 27.8 months (12~36 months). The status of reduction and union were assessed with radiographs. The functional outcome was evaluated by Neer¡¯s rating system.

Results: By Neer¡¯s functional evaluation, 14 cases that bone graft had not been done were above satisfactory, and 7 cases that bone graft had been done was above satisfactory. There was no case of nonunion, nerve injury, avascular necrosis of the humeral head or infection.

Conclusion: We obtained satisfactory results of LCP fixation both with bone graft and without bone graft for fracture of proximal humerus in this study. Bone grafting for bad bone quality and severe bone loss in treatment of proximal humerus fracture using LCP is a reliable method to obtain satisfactory reduction, rigid fixation and early exercise.
